Transformative Healthcare with StationMD
Developmental Disabilities Awareness Month highlights the importance of inclusion, accessibility, and quality care for individuals with intellectual and developmental disabilities. At TNC Community, we are dedicated to ensuring individuals receive the medical care they need—when they need it. One of the most impactful ways we achieve this is through our partnership with StationMD.
Meeting Complex Needs with Specialized Telehealth
The individuals in TNC Community homes have severe IDD diagnoses, and 75% are medically fragile due to complex health conditions. To provide timely and appropriate care, our dedicated Health Team of nurses and healthcare coordinators is available around the clock. However, some situations require the expertise of medical doctors.
When complex medical situations arise, StationMD enhances the high-quality care our Health Team provides. Their board-certified doctors specialize in treating individuals with IDD via telehealth assessments, allowing them to receive expert care from home.
For individuals who rely on wheelchair-accessible transportation, a single medical visit can be exhausting. It often involves multiple transfers, navigating hospital grounds, and long ER wait times. By eliminating unnecessary trips to the ER, we also reduce stress and limit exposure to illness.
Unlike many emergency room physicians, StationMD doctors have specialized training in IDD-related health conditions. Their expertise ensures accurate diagnoses and appropriate treatment, preventing misdiagnoses and repeat hospital visits.
Reducing Visits and Enhancing Comfort
In 2024, TNC Community made 176 calls to StationMD. Of those cases, 171 were treated at home. That means 97% of calls were resolved without a hospital visit, reducing anxiety, discomfort, and unnecessary disruptions for our residents.
For example, Elisha (49) requires 24/7 support in a TNC Community home due to a traumatic brain injury. When she showed signs of an infection, a StationMD doctor quickly ordered a test. Instead of an ER visit, our nurse collected a specimen and took it directly to the lab. This process ensured Elisha received care quickly and comfortably—without leaving home.
In another case, Craig (58), who has Down syndrome and takes multiple medications daily, hurt his foot. StationMD responded immediately. The doctor placed an order for an x-ray, allowing Craig to bypass the ER and go directly to his appointment. This quick action minimized disruptions to his routine and ensured he received the right care.
